I have also acknowledged and consented to have the Kavik Foundation run an unconditional, extensive, profound, and complete background check on me and any family members who will be staying with the future animal that I'm/we're adopting by utilizing the information I've provided above. Any adoption fee charged back after the adoption will result in your adopted animal being confiscated. Submitting this form does not automatically qualify you to adopt your desired animal. Due to our staffing, the reply time after submitting this application may vary. I know that this pet may live 12 or more years,s and I am preparing to give it the medical and emotional care and support it needs. Also, I understand that expenses (i.e., food, medical care, vaccinations, supplies, etc.) may average $1,000 or more annually. Adoptions are based on the best match. Thus, this application may not necessarily result in adopting the animal(s) listed above.
